The patient,\u00a0Smt. Bandakindhi Swapna<\/strong>,38 year old woman a resident of Hyderabad<\/strong>, was suffering from\u00a0severe bioprosthetic valve stenosis<\/strong>\u00a0with preserved left ventricular function, a condition indicating structural valve degeneration. She had previously undergone surgical bioprosthetic aortic valve replacement 8 years ago and recently presented with\u00a0progressive shortness of breath and fatigue<\/strong>.<\/p>\n

Dr. Karunakar Rapolu, Consultant Cardiologist,<\/strong>\u00a0Apollo Hospitals, Jubilee Hills, said<\/strong>\u00a0this case was particularly high-risk and challenging because of the patient\u2019s\u00a0complex medical history<\/strong>, which included\u00a0Bronchial Asthma, Anaemia, Hypothyroidism<\/strong>, and\u00a0Seizure Disorder<\/strong>.\u00a0Despite consulting several leading hospitals across Hyderabad, she was\u00a0denied minimally invasive treatment options<\/strong>\u00a0due to her age and associated high surgical risks.<\/p>\n

Recognising the urgency and intricacy of the case, the\u00a0Heart Team at Apollo Hospitals, Jubilee Hills,<\/strong>\u00a0undertook a comprehensive evaluation involving\u00a0CT angiography and multidisciplinary consultations<\/strong>.\u00a0The team, led by\u00a0Dr. Karunakar Rapolu, Consultant Cardiologist<\/strong>, decided to proceed with a\u00a0Valve-in-Valve TAVI<\/strong>\u00a0 a cutting-edge, catheter-based intervention that eliminates the need for open-heart surgery.<\/p>\n

For the procedure, the team selected the\u00a0Allegra self-expanding transcatheter valve<\/strong>, chosen for its\u00a0superior hemodynamics, repositionability<\/strong>, and\u00a0compatibility with smaller valve annuli<\/strong>, critical for redo procedures within a failed surgical bioprosthesis.<\/p>\n

The procedure was performed successfully via\u00a0transfemoral access under conscious sedation<\/strong>. The new valve was accurately deployed within the existing failed valve with\u00a0excellent post-implant valve function, Minimal transvalvular gradient, no paravalvular leak, and Good coronary clearance<\/strong><\/p>\n

Dr. Karunakar Rapolu, Consultant Cardiologist,<\/strong>\u00a0said the patient experienced an\u00a0uneventful recovery<\/strong>\u00a0and was discharged just\u00a0three days post-procedure<\/strong>\u00a0with marked symptomatic relief, now classified as\u00a0NYHA Class I<\/strong>. At her\u00a0one-week follow-up<\/strong>, she remained asymptomatic with stable valve function and\u00a0improved physical endurance<\/strong>.<\/p>\n
