Smaller Incisions, Faster Recovery: Mini Laparoscopic Surgery Changing Gallbladder Treatment

New Delhi [India], July 10: Gallbladder removal surgery is becoming increasingly patient-friendly as advances in minimally invasive techniques continue to improve recovery and cosmetic outcomes. A newer form of mini laparoscopic surgery, known as the 5-5-2-2 technique, is offering selected patients less postoperative pain, smaller scars and a quicker return to normal activities compared to conventional laparoscopic surgery.

Gallstones are among the most common digestive disorders requiring surgery. While conventional laparoscopic cholecystectomy has long been regarded as the standard treatment, refinements in surgical techniques are enabling surgeons to perform the procedure through even smaller incisions without compromising safety in appropriately selected patients.

One such patient, a 35-year-old woman experiencing recurrent upper abdominal pain due to gallstones, recently underwent mini laparoscopic gallbladder surgery using the 5-5-2-2 approach at Sir Ganga Ram Hospital. Like many patients scheduled for surgery, she was concerned about postoperative discomfort, visible scars and the time needed to resume her daily routine.

According to Prof. Dr. Tarun Mittal, Vice Chairman and Head of Unit, Department of Laparoscopic, Laser and General Surgery at Sir Ganga Ram Hospital, “The procedure represents an important advancement in minimally invasive surgery. Instead of the conventional port configuration, the procedure was performed using two 5 mm ports and two ultra-fine 2 mm ports. The smaller incisions were designed to minimise tissue trauma while allowing safe removal of the gallbladder using specialised mini-laparoscopic instruments.”

The patient recovered well after surgery. She was able to walk within a few hours of the procedure, required minimal pain medication and was discharged early. Within a few days, she resumed light daily activities and gradually returned to her normal routine. At her follow-up visit, the surgical marks were barely visible.

According to surgeons, reducing the size of surgical ports can help lessen postoperative pain, decrease the need for painkillers and improve cosmetic outcomes. However, they emphasise that the technique is suitable only for carefully selected patients, and the choice of procedure depends on factors such as the severity of gallbladder disease, inflammation and the patient’s overall health.

The surgery was performed by a team comprising Dr. Tarun Mittal, Dr. Ashish Dey, Dr. Anmol Ahuja, and Dr. Shresth Manglik.

Mini laparoscopic cholecystectomy using the 5-5-2-2 technique offers several potential advantages, including:

  • Less postoperative pain
  • Smaller and less noticeable scars
  • Reduced need for pain medication
  • Faster recovery
  • Earlier discharge from hospital
  • Quicker return to work and routine activities

Surgeons also note that while robotic-assisted gallbladder surgery has become increasingly available, it generally requires larger ports than mini laparoscopy and may involve higher treatment costs. For selected patients with uncomplicated gallbladder disease, mini laparoscopic surgery can provide a less invasive alternative with comparable surgical outcomes. However, more complex cases may still require conventional laparoscopic techniques to ensure optimal safety.

As minimally invasive surgery continues to evolve, the emphasis is increasingly on improving patient comfort, shortening recovery time and achieving better functional and cosmetic outcomes without compromising the effectiveness of treatment.
