After 40, Your Body Changes Fast: Here’s What Livlong 365 Doctors Say You Must Track Every Year

Posted on October 21, 2026 By

New Delhi [India], October 21: Forty isn’t a dramatic turning point on its own, but it’s roughly where several slow-moving changes in the body start showing up on paper before they show up as symptoms. Blood pressure creeps, bone density starts to dip, near vision blurs, and nutrient levels quietly fall, all without necessarily feeling like anything at all. Livlong 365 breaks down what’s actually worth tracking annually once you cross 40, and why “I feel fine” isn’t the same as “I’m fine.”

Blood Pressure and Blood Sugar: The Two Silent Movers

India’s disease burden data makes the case bluntly. According to a national ICMR-INDIAB study mentioned in a 2026 epidemiology publication, hypertension prevalence among Indian adults stands at 35.5% while diabetes prevalence is at 11.4%, with a further 15.3% in the pre-diabetic spectrum, meaning that nearly half of all Indian adults fall on this spectrum without being aware of it.

  • Both conditions are usually symptomless in early stages; the only way to catch them is a basic BP check and fasting/HbA1c blood test, done annually rather than “when something feels off.”
  • A 2025 National Statistical Office survey found cardiovascular disease hospitalisation touching 13.9% of people aged 45–59 and 7.3% of those aged 30–44 in a single year, evidence that cardiac risk is no longer a “60-plus” concern.

Heart Health: Why the Age of Onset Is Shifting Younger

A late-2025 nationwide survey of nearly 300 Indian cardiologists found that three in four cardiologists now see most of their heart patients under age 50, with over a third reporting cardiac patients as young as 31–40, and pointed to how often early red flags like breathlessness or fatigue get missed or brushed off.

  • An annual lipid profile, ECG, and blood pressure check from 40 onward is the baseline most cardiologists now recommend, rather than waiting for chest pain to prompt a visit.
  • The same survey found treatment costs have risen nearly 65% in five years, another reason catching risk factors early, while they’re still manageable with lifestyle changes, matters financially as much as medically.

Bone Density: Especially for Women Approaching Perimenopause

Bone loss accelerates well before most people think to check for it. An Indian research conducted recently that investigated postmenopausal women revealed that only 18% of them had a normal bone density measurement, while 44.7% registered as having osteopenia and 37.5% suffered from osteoporosis – which is such a situation that even a slight bump could result in a fracture.

  • Clinical guidance in India recommends opportunistic bone density screening for asymptomatic women starting at 40, not waiting until after menopause is complete.
  • Men aren’t exempt either; bone loss in Indian men shows a steady rise from the 40s onward, just at a slower rate than in women.

Eyes: Presbyopia Isn’t Optional Aging, It’s Trackable

Declining ability to see things up close (presbyopia) is another common and neglected post-40 change. According to national statistics, around one-third of Indians aged above 30 years have presbyopia that is not treated with any corrective methods, which means that about 170-180 million people in India are actually changing the position of their phone rather than getting checked.

  • An annual eye exam after 40 does double duty, correcting near vision and screening for early cataract or glaucoma, both of which become more common in this decade.

Nutrient Levels: Vitamin D and B12 Deficiencies Are Common, Not Rare

A 2025 report from the Indian Council for Research on International Economic Relations found that one in five Indians is Vitamin D deficient — a number that climbs further in urban, indoor-heavy lifestyles typical of the post-40 working years. B12 deficiency, tied to fatigue, memory issues, and nerve symptoms, is similarly widespread and easy to miss without a blood test.

  • A simple annual Vitamin D and B12 panel can catch deficiencies before they show up as fatigue, brain fog, or bone pain that gets wrongly chalked up to “just aging.”

Cancer Screenings Worth Starting in This Decade

Screening timelines shift meaningfully after 40:

  • Breast cancer: Growing global evidence now supports starting mammography conversations at 40 rather than waiting until 50, given better outcomes when cancers are caught before symptoms appear.
  • Oral cancer: Particularly relevant in India given tobacco, gutka, and betel-nut use, a simple visual screening by a doctor is quick and often skipped only because it’s not painful enough to prompt a visit.
  • Colorectal cancer: International guidelines have moved the recommended starting age down to 45 amid rising cases in younger adults, a useful marker for when to start the conversation with a doctor.

Dr. Sonal Mhatre, Senior Manager – Clinical Operations, says, “The tests I recommend after 40 aren’t really about finding something wrong; they’re about catching a trend before it becomes a diagnosis. Blood pressure, sugar, bone density, vitamin levels– none of these announce themselves early. That’s exactly why they need to be checked on a schedule, not on a symptom.”

Making the Annual Check-Up Less of a Hurdle

Livlong 365 frames these check-ups as a decade-specific checklist rather than a vague “get healthy” message:

  • Bundling the above tests into a single annual preventive health package rather than treating each as a separate, easy-to-postpone errand
  • Making OPD consultations and diagnostic testing accessible enough that an annual check-up doesn’t require a full day off work
  • Framing this decade’s check-ups as course correction, not crisis management — most of what shows up at 40 is manageable if caught early

Turning 40 doesn’t change things overnight, but regular tests and check-ups can help you stay one step ahead, before any health concern gets worse.
